Re-Evaluation
Every student requiring a reevaluation must have a on-line referral completed.  The process is shorter for a reevaluation than an initial. 

It is recommended that the case manager initiate the on-line assesssment referral process and create a referral team for a student one week prior to the student being brought to the child study team.  This allows all teachers to log on given current information that can be considered at the child study meeting.  Additional information can be added during the child study meeting leading to the next step of creating the re-evaluation plan.  At this time, parent notification should take place to allow for current parent concerns and provide them the opportunity to attend the child study meeting.  Parent concerns should be noted on the "Concerns" tab and then be available to enter into the ESR.


The purpose of using the on-line referral process for reevaluations is to provide the district with data about amount of assessment being completed by staff and the outcomes of those assessments.  It gives an accurate accounting of the students that require ongoing testing.

    BEGINNING THE REFERRAL FORM

    The menu of referral options has two clusters of statements. The second cluster of five items pertains to student referrals for re-evaluations. CAREFULLY read the statements in the second cluster and select the option that matches the student's circumstances. The program will choose the form that needs to be filled out, Initial or Re-evaluation, depending on the particular statement that you choose.

    Example: "Student has a special evaluation from another Minnesota school district that is NOT accepted bu SPPS" would lead you to the  Initial Evaluation Form to be completed.

    *You must select your school to do a student search.
    * Search for a student and click on desired result.
    You can only view students for whom you have Campus
    rights.

    TIP! Use only the minimal student information in the search that will identify the student, i.e. student number, or last name.  If you are a classroom teacher you can use the section search to find a student in your class.

    RE-EVALUATION TEAM


    From the list of staff, choose those that you want to give input into current concerns and status.  Click on each member and then "Add Staff".  More than one can be added at a time (as many as needed).

    For Example:  A seventh grade student is being referred for his 3-year special education re-evaluation.  The case manager would put the student's general education teachers and all of his special education service providers on the referral team in order to get comprehensive input on these student's current level of performance and concerns.


    You can choose from "all district staff", which is very lengthy, or "special education" staff which is less lengthy.

    TEAM EMAIL is a handy way to notify staff of their membership and responsibilites.  Be sure to put a check in the blue box for each person an email is to be sent.

    On Reassessment, explain continuing or new concerns regarding special education services that require testing.

    or


    Is data sufficient to document that the student continues to have a disability and meets continuing criteria for service.

    For confidentiality, specific diagnoses should not be indicated.on this form, but brought to the child study team.  The nurse will include necessary information within the ESR document.




    It is reaquired to note the date of the parent notification using the calendar feature.  The concerns parent expressed duing the interview should be noted on this page.

    ell evaluation procedures
    ELL EVALUATION PROCEDURES

    If there is a history of a language other than English as documented on the initial evaluation, on Campus, or in recent information obtained from the parents, then consideration must be given to ELL factors in planning the re-evaluation. This includes students who speak an African dialect, such as Nigerian English or Liberian English and students adopted from another country after the age of 12 months.

    To activate the process for involving the Special Education ELL assessment team in a student's re-evaluation, complete fill out the information under the ELL tab after the other parts of the online re-evaluation form is complete. Check the box for "Send referral to the SE ELL Assessment team". When you click on the "Save" button, the referral will be sent to Carol Dady in the SE office, the referral logged in and printed off for the ELL Assessment team. You will then receive an email back letting you know that the referral has been received and the SE ELL team process activated.

    final determination
    FINAL DETERMINATION OF DISABILITY

    **This is a Child Study process and might well be scheduled biweekly to stay current with those in assessment.

    **When it is time to return to the evaluation to enter the final outcome of the assessment, search for the student, and click on the blue number which will take you back to the referral.  Then click the last button "Final Determination of Referral". (ID 4, ID 113 below)


    After the Re-Evaluation is completed and IEP written, go to "Final Determination of Referral" and document the results of the assessment.  This assures the student will be counted when the district accumulates data on the efforts of the child study team.



    Be sure to click the "Referral is Complete" checkbox to the right to close the evaluation process.


    These are the Federal Settings as described numerically by the state:

    O1 Outside of the regular classroom less than 21% of the day
    02  Resource room between 21% and 60% of the day
    03  Separate classroom more than 60% of the day
    04  Public day school facility greater than 50% of the day
    05  Private day school greater than 50% of the day
    06  Public residential facilities greater than 50% of the day
    07  Private residential facilities greater than 50% of the day
    08  Homebound / hospital placement


    11  Early Intervention:  Developmental Delays - Self Contained Classroom or setting
          with 50% or more children with IFSPs / IEPs
    12  Early Intervention: Typically Developing - Regular preschool setting with less than
          50% children with IFSPs / IEPs
