    STUDENT SEARCH

    *You must select your school to do a student search.
    * Search for a student and click on desired result.
    You can only view students for whom you have Campus
    rights.

    TIP! Use only the minimal student information in the search that will identify the student, i.e. student number, or last name.  If you are a classroom teacher you can use the section search to find a student in your class.



    My Referrals: Referrals you personally initiated.


    Team Referrals: Referrals you are named as referral Team Member.

    My Sections: Find a student based on your class section student list.

    BEGINNING THE REFERRAL FORM 

    CAREFULLY select one option from the menu below.

    The menu of referral options has two clusters of statements. The first cluster of three items pertains to student referrals for initial evaluations. CAREFULLY read the statements in the first cluster and select the option that matches the student's circumstances.

    Example: "Student has a special evaluation from another Minnesota school district that is NOT accepted by SPPS" would lead you to the  Initial Evaluation Form to be completed.

    STUDENT REFERRAL INFORMATION

    Each GRAY tab in the top menu contains fields that must be completed.  Click each tab separately .  You may return to a tab at another time.  Click the SAVE button at the bottom of the window or your entry information will be lost.

    The GREEN tabs do not need to be completed by the referring teacher, but are used by building principal and child study member.
    RED text indicates required fields.

    building assessment teams
    BUILDING ASSESSMENT TEAMS

    You will need to create a "referral team" to complete the form.  This is a group of teachers and related service providers that you want to log on to the website to add their input to the referral.


    Example: a fifth grade teacher is referring a student for an initial evaluation.  (S)he might add the physical education teacher, the nurse, and another fifth grade who has been involved for reading pre-referral interventions.


    Click on the box to the left of the team member to identify those participating.  Click “Add Staff” and they will appear in the window labeled “Current Team”



    By clicking Team Email, you get a window that will allow you to send notification of a referral in process for those you have selected.  REMEMBER to click the boxes to the right of the names to identify those to whom you are sending the notice.

    STUDENT CONCERNS

    Complete the checkbox for the student being referred.

    List general concerns at this time.   You will be given opportunity to state additional information in areas where there are concerns as you go through the process.

    documenting interventions
    DOCUMENTING INTERVENTIONS

    Click the “Add an Intervention”
    Describe the first intervention that was implemented.  Click SAVE.

    The intervention should include a description of what was implemented.  Use the calendar feature to indicate the start and end dates.   Include a summary of the data collected in the results section.    Then choose a statement from the list that conveys the outcome of the intervention.


    Click on “Add an Intervention” again to Complete the second Intervention.  SAVE.

    STUDENT INFORMATION TABS

    After interventions are complete, proceed through the student information tabs labeled:

        Intellectual Functioning
        Academic Performance
        Communication
        Sensory/Motor
        Health (completed by nurse)
        Emotional/Behavioral
        Functional Skills
        Transition (turning 14 yrs and over)

    NOTE: the default is "Unknown".  Be sure to CHANGE each statement to indicate which description is accurate.  "Unknown" is not an accurate description of any of the statements, with the exception of  Transition (applies to 14 yrs and older).

    Be sure to click SAVE after each window.

    The STANDARDIZED ASSESSMENTS require no input.  The previous two years of testing will automatically display.

    SUBMIT TO PRINCIPAL

    At any time, a team member can go to the referral and access the various tabs to enter information.  Click on the blue ID number (ID4 or ID113 pictured below).

    When all referral team members have logged on and the  referral is complete, the referring teacher goes to student search, select the student being  referred, and click the "Report" button to create a report and print to give the principal for approval. 

    To print the report, search for the student and click the blue "Report" button which will put the document in print format:


    The Child Study team will complete the GREEN tabs as the referral continues through the process.

    CHILD STUDY TEAM INSTRUCTIONS

    The Child Study Team will receive the printed referral from the building principal indicating approval to consider the referral for assessment.  The on-line referral must be updated with the principal's decision, and proceed to the GREEN tabs as the process continues.

    Referral Status - Check "Principal's Approval" - SAVE
    Child Study Team Action - Check one of the three options and SAVE (note - checking Referal to ELL will result in referral going to the ELL Assessment Resources).

    After the evaluation and IEP process is complete, the evaluation manager should return to the "Final Determination of Referral" page and complete each of the four menu items:

    Eligiblity

    Primary Disability

    Secondary Disability (if applicable)

    Federal setting

     

    Be sure to click the "Referral is Complete" checkbox to the right to close the evaluation process.


    These are the Federal Settings as described numerically by the state:

    O1 Outside of the regular classroom less than 21% of the day
    02  Resource room between 21% and 60% of the day
    03  Separate classroom more than 60% of the day
    04  Public day school facility greater than 50% of the day
    05  Private day school greater than 50% of the day
    06  Public residential facilities greater than 50% of the day
    07  Private residential facilities greater than 50% of the day
    08  Homebound / hospital placement


    11  Early Intervention:  Developmental Delays - Self Contained Classroom or setting
          with 50% or more children with IFSPs / IEPs
    12  Early Intervention: Typically Developing - Regular preschool setting with less than
          50% children with IFSPs / IEPs
    13  Early Intervention:  Home
    14  Early Intervention:  Hospital Patient
    15  Early Intervention: Residential Facility
    16  Early Intervention: Service Provider Location
    17  Early Intervention: Other Setting


    31  In the regular early childhood program at least 80% of the time.
    32  In the regular early childhood program 40% to 79% of the time.
    33  In the regular early childhood program less than 40% of the time.
    41  Early childhood - Separate Class (Class has less than 50% children without
          disabilities)
    42  Early childhood - Separate School (Public or private day schools designed
          specifically for children with disabilities) 
    43  Early childhood- Residential Facility (Public or private residential schools or
          residential medical facilities)
    44  Early childhood - Service Provider Location (Receives special education and related
          services from a service provider, and child does not attend an early childhood
          program or a special education program)
    45  Early childhood- Home (Receives special education and related services in the
          principal residence of the his/her family or caregivers, and who did not attend and
         early childhood program or a special education program).
