Use of Conditional Procedures/Behavior Intervention Plan
If the planned use of a conditional procedure is going to be used with a student with a disability, the following items must be included in the student's IEP:

1.  The frequency and severity of the target behavior(s) for which the conditional procedure(s) is being used must be addressed under the present level of performance (PLEP) in a goal area related to behavior.

2.  Two positive interventions which were implemented and the effectiveness of each must be described in the PLEP;

3.  State the conditional procedure(s) that will be used in the Adaptations section of the IEP including:

     -Manual restraint

     -Mechanical or locked restraint

     -Time Out:

       a.) Isolation room/seclusionary time out

       b.) Removal from planned instruction for 30+ minutes.
