Parent Request for an Evaluation 1. A parent has the right to request an evaluation for their child. The evaluation request can be made in written form or verbal form.. 2. The Child Study Team must respond to the parent within 10 school days indicating that the request has been accepted or refused. · When a request has been accepted, the Child Study Team must seek input from the parent to develop an evaluation plan, complete the Notice of Evaluation/Reevaluation form, and obtain the parent’s written consent before proceeding with the evaluation. · When a request has been refused, the Notice of Refusal of Parent Request form must be sent to the parent indicating the reasons that the evaluation has been refused. 3. Before a decision about the request is made, the Child Study Team may ask the parent to allow the school to attempt interventions in general education before considering the student for an evaluation. However, the parent has the right to waive this request. |
