Other Health Disability Minnesota schools are serving students with an extraordinary range of chronic or acute health conditions. Medications, treatments, therapies, and repeated hospitalizations can impact a student’s ability to learn and function at school. A student with such a condition may be considered for special education under the Other Health Disabilities (OHD) category. Minnesota State Website OHD

* Used for observations and interviews
* Used for both teacher interview, student observation, and parent interview

*Used to meet the requirement for documentation of a medical diagnosis
* Needs to be completed and signed by a licensed physician
* Needs to be accompanied by a signed parental permission form
* Must be dated within the previous 12 months for an initial evaluation

* Used to meet the requirement for documentation that DSM-IV criteria were met in making the diagnosis
* DSM-IV criteria documentation must be provided by either a licensed physician or a mental health or medical professional licensed to diagnosis the condition
