Notice of a Team Meeting
The purpose of this form is to invite to and inform parents of a meeting proposed by the school district. This notice also provides parents with the names and titles of persons expected to attend the meeting.

Purpose: Check the appropriate box or boxes to indicate the purpose of the meeting.
