Notice of Transfer of Parent Rights This notice must be sent to both the parent and their child who is about to turn 18. The notice explains that parent rights will transfer to the student, including incarcerated youth, when s/he turns 18. Complete the form by including the name of the parent(s), the name of the student and the student’s birthdate. |
