Documentation for all Speech/Hearing, Nursing, Mental Health(includes social work and psychology), Physcial Therapy, Occupational Therapy, Interpreter
Document each individual service, including Date of Service, Duration of Service, Service Description, Progress, and Additional comments as necessary.
Transportation
Transportation is a covered service if the child requires a special adaptation to the bus, which may include:
- Wheelchair lift
- Special harness
- Safety vest
- Special car seat
- One to one
Curb to curb is not listed as special transportation for billing purposes.
If students are receiving any special transportation as listed above, documentation is required in the adaptations section of the IEP.
Examples:
"Student requires a harness on the bus for safety".
"Student requires a one to one aide on the bus for redirection and intervention for behavior".
Student is in a wheelchair which requires use of a lift bus".
Campus Instructions for adding special transportation to the IEP.
1. Create Education Plan
2. Click on Adaptations under SPPS Plan Outline
3. Click on Adaptations
4. Click on the white envelope icon under"Adaptations in General and Special Education
5. Click on +signIEP Adapatation Gen & Spec. Ed. under Template Bank
6. Click on Personal Care Services & Transportation
7. Choose all that apply
Highlight what does not apply and delete
8. Save and continue
In order to bill for special transportation, documentation is required that verifies that the child actually rode the bus to/from school and home. Notify the billing office, 767-8189, or 767-8191, and they will send the transportation logs for the student. The instructions for completing the Trip Log are listed below
